Students Code of Conduct Agreement
The Code of Conduct Agreement is to be read and filled out by students attending Student Ministry trips.
** Parents/guardians are not to complete the form for the student but are to sign alongside their student.
Once you have completed the forms above, please save each one with your student's name as part of the file name and email them to bradon@fbckosciusko.com.
